Headquartered in Albuquerque, NM  Journeys provides quality services under the Developmental Disabilities Waiver for the State of New Mexico Department of Health Developmental Disabilities Support Division.  Journeys offers the following services in the following counties of New Mexico:

Community Integration (Bernalillo and Sandoval)

to promote self-determination, increase independence and enhance the person's ability to interact with and contribute to their community.

Outlier Day Habilitation (Bernalillo and Sandoval)

intended to meet the needs of individuals with severe, chronic needs.

 

Family Living  (Bernalillo, Sandoval, Santa Fe, Los Alamos, Rio Arriba, Mora, San Miguel, Socorro and Valencia)

intended to provide a safe home where the individual lives with the paid direct support provider, i.e., this is not a shift staffing model.

 

Substitute Care  (Bernalillo, Sandoval, Santa Fe, Los Alamos, Rio Arriba, Mora, San Miguel, Socorro and Valencia)

intended to supplement and to provide assistance to the Family Living Providers to be able to relinquish their responsibilities to a temporary provider.

Training (Statewide)

intended to provide staff with the necessary tools and knowledge to successfully provide quality services.
